OFFICE POLICIES
We ask that you please be on time for your appointment, as one late patient creates a cascade of late appointments.
If you arrive more than 10 minutes after your scheduled appointment or run more than 10 minutes over your scheduled appointment, you will be charged an additional $25 fee and may be rescheduled for a later day at the discretion of the provider. This fee is not billable to any insurance and will be charged to you personally.
When you have an appointment, we request that you please only discuss the medical conditions relating to the scheduled patient. If another child or family member needs an appointment, we will be happy to schedule an appointment for them.
If you miss 2 consecutive appointments without notifying our office, all services will be suspended. You will receive written notice via email and/or US Postal service to notify you of the suspension.
Failure to pay the membership for more than two months is grounds to suspend service. You will be notified via email and/or US Postal service of the suspension and thirty (30) calendar days to make your account current before it will be terminated.
Provide at least one week’s notice that you need a refill of any prescription. Do so in writing via the patient portal addressed to your provider’s medical assistant or select “prescription refill” in the Klara app.
If you have been waiting more than 15 minutes for a scheduled appointment, please notify the front desk or nurse.
School/work absentee notices will be provided at the provider’s discretion.
SCCIM now requires patients to utilize the Patient Health Records (PHR) portal. You will receive an invite when you schedule your initial consultation.
For your convenience, we do our best to stock the supplements that our providers recommend. You are under no obligation to purchase them from us, but we accept no responsibility for supplements purchased from any other parties or failure to follow the directions of your medical providers.
We do not provide superbills or diagnosis codes to cash-pay patients. If SCCIM is out of network with your insurance we can supply you with a superbill to file for reimbursement. Superbills will be given out at the discretion of our medical biller.
Unopened products have a 30-day return policy with proof of purchase.
Primary Care Membership
We utilize a membership plus fee-for-service model. This means that in addition to charging a fee for office visits, we also charge a nominal monthly fee and require that all patients be enrolled in the membership of our practice.
Our $30/month per person membership includes:
The ability to schedule appointments.
Communication with your practitioner via email or text. The preferred method of communication will be discussed at your initial consult. (*If your message is too complicated to reply to in text or email, your provider will direct you to schedule an appointment to discuss it in person.*)
The membership fee is:
Automatically drafted on the 25th of every month
Billed for the 25th of the previous month through the 24th of the current month. We do not bill in advance for monthly service.
Our family capped rate at $120/month includes:
Two adults above the age of 18 and related children under 21 years of age. Children ages 21 and over will be counted as additional adults. Adult children who marry will be required to begin their own membership.
Additional adults may be added for $30/each at the discretion of the practice and are not included in the capped rate.

The below fee schedule is applicable to all new appointments effective immediately. This is a general list of our most common charges and may not include fees associated with a visit due to extenuating circumstances. Pricing is determined by the provider at the time of service and is based on the time and complexity of each visit.

30 Minute Follow Up

Includes a 30-minute comprehensive office visit to adjust or evaluate an ongoing treatment plan.
Sick Visit

Includes a 15-30 minute focused visit to address an immediate illness.
Well Baby Visits

These visits are conducted in a group “centering” style appointment.
Annual Wellness Exam

Includes sports or work physicals, these are routine wellness exams.
Health Coach Appointment
$50.00
Health coaches are an integral part of lifestyle modification. They are here to support the changes that your providers may make. The health coaches will also help answer questions as you progress through your treatment plans.
Telemedicine Appointment
Telemedicine appointments are billed at a variety of rates depending on the complexity of your clinical case and the length of the appointment. Any appointment that exceeds the 30-minute appointment or reflects a higher complexity level will be billed accordingly.
Prenatal Exams
Prenatal exams are billed through insurance as a standard visit based on the complexity of the appointment. Cash rates are available and subject to billing guidelines and regulations.
Release of Medical Records
Requests for medical records released to individuals will be billed at $10-25 depending on the complexity and volume of the records. You will be notified of the charge prior to running it.
Some in-office procedures will incur additional charges but will be discussed prior to starting.
IF YOU NEED TO CANCEL A SCHEDULED APPOINTMENT, YOU MUST DO SO 24 HOURS PRIOR.
Text, email, or call to let us know that you no longer require your scheduled appointment so that someone else can be seen.
If you miss a scheduled appointment without notice, you will be charged for the visit.
Membership Cancellation Policy:
There is no contract or minimum enrollment period. However, if a member cancels his/her membership there is a 9-month waiting period to reenter the practice. If for some reason the member needs to re-enroll sooner, there is a $100 readmission fee per patient.
If you wish to cancel your membership, you must email notice that you wish to do so prior to the 5th of the month. Cancellation requests after the 5th of the month will be charged half of the full membership and all emails received after the 18th will be charged for the entire month.
If you cancel your membership or your membership is canceled with a balance due, we reserve the right to refer your past due balances to collections.
Cancellations MUST be done in writing to membership@sccimed.org.
Emailed cancellation requests need to include the names and birth dates of the membership being canceled and a brief explanation of the termination.
