Financial Policy and Disclosure
This financial policy and disclosure is to help us provide efficient and effective healthcare services.
Patients are responsible for all services provided by South Carolina Center for Integrative Medicine.
Self-Pay/Cash-Pay Policy
• Self-pay patients are required to pay for the office visit and any additional balances after their visit, in full. No billing or payment plans will be authorized.
Insurance Policy
If you have private insurance, it is our policy to file for insurance as a courtesy to you.
It is the patient’s responsibility to ensure that we have the most accurate and complete
insurance information at the time of any rendered services.
Any services rendered that are not covered by the patient’s insurance are the responsibility of
the patient. Non-covered services include but are not limited to: medications, supplements,
TENS Unit therapy, and health coach appointments.
Deductibles, Co-payments, and coinsurance will be provided prior to any services being
rendered. Balances due will be collected after the appointment.
If we need assistance contacting your insurance company for payment, we expect that you will
be of assistance.
If your insurance company fails to pay within the contracted time allotted, you accept responsibility for the balance due.
Overdue Balances
Overdue balances will be reported to you via email and you will be given thirty (30) calendar days to make payment.
Failure to make payment on an overdue balance will result in your expulsion from this practice and potentially collections efforts.
Divorce or Custody Cases
The parent or legal guardian who brings a custodial patient into our office is financially responsible, regardless of the provisions of divorce decrees, custody arrangements, or insurance holder status.
Patient Responsibility
Provide current and updated information on yourself and your insurance company.
Present updated insurance information or photo identification card if/when changes are made.
Make the appropriate payment at the time of service based on your insurance or self-pay status.
Do not discuss financial details with your provider. This would distract them from providing the
best quality healthcare possible to you. Account information should be limited to the front desk or administrative personnel.
